Subrecipient Community Health Worker (CHW) Program
The Alaska Primary Care Association is seeking 15 Subrecipients to partner with APCA in recruiting, hiring, and training Community Health Workers (CHWs) to address health disparities in rural Alaska. Subrecipients will employ and mentor one CHW to support underserved communities and enhance healthcare outcomes, particularly for individuals at higher risk for severe COVID-19 illness.
